To ensure the safe and responsible use of stimulant medications, all patients receiving these prescriptions are required to enter into a medication management contract with our practice. This contract will cover the following key areas:
-
Medication Adherence: Patients agree to take their medication as prescribed and to follow the prescribed dosage and schedule.
-
Prescription Refills: Refills will only be provided according to established procedures, and early refill requests will not be granted.
-
Controlled Substance Agreements: Patients agree to not obtain controlled substances from other physicians, or pharmacies, without the knowledge of their prescribing physician.
-
Appointment Attendance: Patients agree to attend all scheduled appointments for medication monitoring and evaluation.
-
Drug Testing: Patients may be required to undergo periodic drug testing to ensure compliance.

By signing this contract, patients acknowledge their understanding of these requirements and agree to comply with them. A copy of the signed contract will be provided to the patient for their records.
Number of People with Patients
To maintain a comfortable and efficient environment for all patients, we kindly request that only the patient and up to two adults attend the appointment. This helps minimize distractions and ensures adequate space.
