Office Policies

Missed Appointment Policy

We understand that sometimes unexpected events occur, making it difficult to keep scheduled appointments. However, missed appointments or late cancellations significantly impact on our ability to provide timely care to all our patients.

Therefore, we require at least 24 hours of notice for any appointment cancellation or rescheduling. If you miss your scheduled appointment, there will be a $75 fee.  If you cancel with less than 24 hours' notice, you may be charged a $25 fee for first time, $50 fee for subsequent appointments, and beyond that, possible discharge

Stimulant Policy

To ensure the safe and effective management of stimulant medications, all patients prescribed these medications are required to have two in person appointments per year. These appointments allow us to monitor your progress, address any concerns, and ensure continued appropriate medication management.

Refill Policy

To ensure timely medication refills, please request refills at least five (5) business days in advance. Refills requested with less than five business days’ notice may not be processed promptly. If a scheduled appointment is missed, a $10 fee will be charged for any associated medication refills.  Refills can be requested through phone and patient portal.

Stimulant Medication Management Contract Policy

To ensure the safe and responsible use of stimulant medications, all patients receiving these prescriptions are required to enter into a medication management contract with our practice. This contract will cover the following key areas:

  • Medication Adherence: Patients agree to take their medication as prescribed and to follow the prescribed dosage and schedule.

  • Prescription Refills: Refills will only be provided according to established procedures, and early refill requests will not be granted.

  • Controlled Substance Agreements: Patients agree to not obtain controlled substances from other physicians, or pharmacies, without the knowledge of their prescribing physician.

  • Appointment Attendance: Patients agree to attend all scheduled appointments for medication monitoring and evaluation.

  • Drug Testing: Patients may be required to undergo periodic drug testing to ensure compliance.

  • Lost or Stolen Medications: Patients agree to report any lost or stolen medications immediately.

  • Potential Consequences of Non-Compliance: Violations of the contract may result in discontinuation of stimulant medication prescriptions.

  • Safe Storage: Patient agrees to keep medication in a safe place, out of reach of others.

  • Diversion: Patient agrees to not share, sell, or in any other way divert the medication.

By signing this contract, patients acknowledge their understanding of these requirements and agree to comply with them. A copy of the signed contract will be provided to the patient for their records.

Number of People with Patients

To maintain a comfortable and efficient environment for all patients, we kindly request that only the patient and up to two adults attend the appointment. This helps minimize distractions and ensures adequate space.

Payment Policy

All copayments are due at the time of service. We accept credit and debit card. Thank you for your cooperation.

Benzo Policy

Due to concerns regarding safety and potential for dependence, our practice has a policy of not prescribing benzodiazepine medications.
